\n \n\n \n Shift : Days/M-F, hours sometimes vary. Possible once a
month evening. \n \n\n \n Location: Main site: Texas Health Huguley
(11801 South Freeway, Burleson, TX 76028, Additional site: Texas
Health Mansfield (2300 Lone Star Road, Mansfield, TX 76063) \n \n\n

\n The role you'll contribute: \n \n\n \n Responsible for
conducting individualized needs assessments, planning and providing
individualized and group instruction in diabetes self-management
education/training and meal planning and Medical Nutrition Therapy.
\n \n\n \n The value you'll bring to the team: \n \n\n \n
Responsible for conducting individual assessments and other aspects
of diabetes self-management skill based on the needs assessment. \n
\n\n \n Documents in the permanent education record all aspects of
the education services provided. \n \n\n \n Provides and documents
follow-up for patients who receive education. \n \n\n \n Assists in
planning, implementation and evaluation of the diabetes patient
education program. \n \n\n \n Interacts with other health
professionals as a productive team member \n \n\n \n Promotes the
diabetes center services through participation in community
activities \n \n\n \n Accountable for assuring the diabetes diet
instruction complies with national standards. \n \n\n \n The
expertise and experiences you'll need to succeed: \n \n\n \n
Minimum qualifications : \n \n\n \n\n \n Minimum of a Bachelors
degree with a major in Food / Nutrition required. \n \n Registered
by the Commission on Dietetic Registration of the American Dietetic
Association required. \n \n Licensed by the Texas State Board of
Examiners of Dietitians preferred. \n \n National certification as
a Diabetes Educator (CDE) preferred. \n \n\n \n\n \n This facility
